The information below pertains to the following license types:


Ophthalmic Dispenser
Renewal Fee   Branch Renewal Fee   **Expiration Date
$340   $190   December 31, 2011
** If you renew after this date, a late fee of $100.00 will be added to your renewal fee.
  • Be advised that after January 31, 2012, you will not be able to renew on-line and must contact your licensing Board/Committee for reinstatement procedures and associated fees.
  • Be aware that it may take 10 – 15 business days to receive your license.
  • The Division of Consumer Affairs is pleased to announce that administrative efficiencies have allowed us to extend a partial credit on the renewal fee for the current renewal period of January 1, 2012 through December 31, 2013.
