A pharmacy that dispenses controlled dangerous substances (CDS) within New Jersey is required to obtain a New Jersey CDS registration. If appropriate, please complete the initial “Dispenser – Pharmacy” application found at https://www.njconsumeraffairs.gov/dcu/Pages/applications.aspx.

Please note, when applying for a CDS registration an out-of-state pharmacy must use the same address of record filed with the New Jersey Board of Pharmacy. If you submit your application by email, an invoice which will include instructions for submitting payment, will be sent to your recorded email address. Please review the application instruction sheet for additional information and instructions. For additional questions regarding a CDS pharmacy registration, please email CDS@dca.njoag.gov.

Mid-level Practitioner CDS Registration Renewal
As detailed in Executive Order #112 and DCA-AO-2020-02, the requirements for an APN to enter into a joint protocol with a collaborating physician and a PA to enter into a signed delegation agreement, are suspended for the duration of the state of emergency declared in response to the COVID-19 pandemic. Renewal of a midlevel CDS registration during the state of emergency is permissible and questions regarding the existence of a joint protocol or delegation agreement should be answered truthfully. Following the state of emergency, midlevel practitioners must enter into a joint protocol or delegation agreement to continue to prescribe and/or administer controlled dangerous substances.

Out-of-State Pharmacy CDS Registration

Out-of-state pharmacies that dispense controlled dangerous substances within New Jersey are required to obtain a CDS registration. The application will be available on the “Applications and Forms” section of this page in the near future, at which time you must apply. To be placed on an email list to receive updates on this matter, please email CDS@dca.lps.state.nj.us. Optometrist CDS Registration Update

The Optometrist Controlled Dangerous Substances (CDS) Registration has been updated to read “SCH II CDS NOT VALID, EXCEPT FOR HYDROCODONE PRODUCTS”, in accordance with N.J.A.C. 13:38-2.4(h).   Optometrists who maintain an active CDS registration will receive an updated registration within the next month.   Once the new CDS registration is received, registrants should destroy the old CDS registration.

CDS Registration Fee Increase

On February 19, 2019, the Drug Control Unit increased the Controlled Dangerous Substance (CDS) registration fees. Note that the CDS registration fees in effect up through February 18, 2019 will be accepted through March 19, 2019.

Revision of Healthcare Facility New Jersey Prescription Blanks

Due to changes to the laws governing the prescription authority for Optometrists and Physician Assistants, the New Jersey Prescription Blanks Program revised the Healthcare Facility (HCF) New Jersey Prescription Blank (NJPB). Through March 31, 2017, practitioners may write valid prescriptions on either the old or the new healthcare facility prescription blanks. Starting April 1, 2017, practitioners must use the new healthcare facility blank to issue prescriptions for their patients.
