Applications and Forms


The State Board of Social Work Examiners approves individuals for certification and licensure to practice social work in the State of New Jersey. There are three levels of social work practice: certified social workers (CSW), licensed social workers (LSW) and licensed clinical social worker (LCSW).

  • If you have a Master's Degree in Social work from an educational program accredited by the Council on Social Work Education (C.S.W.E) or in candidacy for accreditation by the C.S.W.E. and have two years of full time clinical social work experience subsequent to earning the Master's Degree.

  • If you have a Master's Degree in Social work from an educational program accredited by the Council on Social Work Education (C.S.W.E) or in candidacy for accreditation by the C.S.W.E. and DO NOT have two years of full time clinical social work experience subsequent to earning the Master's Degree.

  • If you have received a baccalaureate degree in social work from an educational program accredited, or in candidacy for accreditation, by the Council on Social Work Education; or you have an official transcript indicating that you acquired a baccalaureate degree prior to April 6, 1995, from an accredited institution of higher education in one of the following fields: guidance and counseling, human services, marriage and family counseling, vocational/disability rehabilitation, psychology, sociology.

Instructions to submit documents during the public health emergency.

The Board staff continues to work remotely. In order to prevent undue delays of the processing of your application or any other request, we ask that you email all applications, name change requests, and verification of licensure requests, to DCA_Social_Work_No_Reply@dca.lps.state.nj.us, even if you have already mailed in your request. Please see below instructions for sending documents.



Applications

  1. Scan and email application, including all supporting documentation, to
    DCA_Social_Work_No_Reply@dca.lps.state.nj.us.

  2. Third-party documents, such as license verifications and official school transcripts, must be received directly by us. If you are unable to have third-party documents emailed directly to us, please include this information in the body of your email. These documents are to be emailed to DCA_Social_Work_No_Reply@dca.lps.state.nj.us.

  3. If you do not have a copy of your notarized application and need to complete a new one, you may attach the Application Attestation form in lieu of a notary.

  4. Once the initial application is processed, you will be sent an invoice to mail in your payment and/or instructions to submit documentation that may be outstanding.

  5. All subsequent inquiries should be sent to socialwork@dca.njoag.gov.



License Verifications

  1. Reach out to the requesting board/jurisdiction and obtain an email address to where we can send electronic verification. We can only provide electronic verification at the time.

  2. Scan and email any document that you may require us to fill out for verification to DCA_Social_Work_No_Reply@dca.lps.state.nj.us.

  3. Include the following in the body of the email: name, license number, and mailing address.

  4. Once the request is processed, you will be sent an email with an invoice and instructions to mail in your payment.

  5. All subsequent inquiries should be sent to socialwork@dca.njoag.gov.



Name Change Requests

  1. Fill out a Change of Name Form. 

  2. Scan and email form and all supporting documentation to
    DCA_Social_Work_No_Reply@dca.lps.state.nj.us.

  3. All subsequent inquiries should be sent to socialwork@dca.njoag.gov.



PLEASE NOTE

DCA_Social_Work_No_Reply@dca.lps.state.nj.us is used to receive application documents, license verification documents, and name change request documents. Miscellaneous inquiries should be directed to socialwork@dca.njoag.gov.
